Project Access Clark County - Patient Care Coordinator *New Posting
The Patient Care Coordinator facilitates and coordinates specialty care for uninsured patients admitted into the program. This position manages patient information and referrals; coordinates specialty appointments and the aftercare required to meet the patient’s needs; tracks patient data; maintains ongoing verbal and written communication with volunteer community providers and works as member of the care coordination team within the Free Clinic as well as with the regional Project Access program (Portland metro area).

Three (3) AmeriCorps Positions
The AmeriCorps member is a 10.5 month, full-time position with a nominal stipend for basic living expenses. The AmeriCorps 2010-2011 service year is September 1st - July 31st and requires a total commitment of 1700 hours.

1. Volunteer Program & Project Access Clark County
(This position provides coordination for two programs.)
Program Coordinator - Volunteer Program (65%)
The Program Coordinator will work with
existing volunteers and will provide volunteer scheduling support to program
leaders. The member will act as the primary facilitator of new volunteers, from
application, interview. and enrollment, to job placement and orientation. They
will act as a liaison with the public to communicate patient need and volunteer
opportunities—this will involve speaking to potential volunteers at volunteer
fairs, over the phone, by email, and through other media.
Program Assistant - Project Access
Clark County (35%)
The Program Assistant will help
support the program through the receiving of referrals and the screening &
eligibility process. Inputting referrals into the database, putting case review
forms on and in Medical Review folder, updating the tracking log, contacting of
patients, working with community partners regarding screening of patients,
processing enrollment applications, process any follow-up from the Medical
Review, returning patient phone calls and dealing with phone messages. Other
duties as assigned.
2. Dental Program & Project Access Clark County
(This position provides coordination for two programs.)
Program Coordinator - Dental Program (65%)
The Program Coordinator is
responsible for the
coordination of dental clinics with Van Coordinator, patient management,
communication with patients, and community outreach.
Program
Assistant - Project Access
Clark County
(35%)
The Program Assistant will provide support inputting provider
information, updating the web-site, closing patient files, creating newly
enrolled patient files, compiling and completing reports, taking meeting
minutes, updating forms when needed, processing mail, contacting DSHS to
check the status of patients in their system. Other duties as assigned.
3. Medical Program
This position is a full time position in one program.
Program Coordinator (100%)
The AmeriCorps Program Coordinator supports overall
clinic goals by assisting in the organization and implementation of our Vision
and Immunization clinics, Urgent Care Clinics, and transitional diabetes
program. The Vision and Immunization clinics are two essential clinic programs
– each with exciting growth opportunities. The AmeriCorps member will gain
experience in understanding how health clinics function and develop
professional skills that can be used in future positions. Urgent care clinics
are a core program of the Free Clinic, and utilize various volunteers to
provide medical care to patients with urgent health concerns. The AmeriCorps
member will also have the opportunity to coordinate a new diabetic program
which seeks to provide a transitional resource where patients’ diabetes is
stabilized before they see an endocrinologist.
