Project Access is a Free Clinic program developed to coordinate donated specialty and hospital services for low-income, uninsured people. Over 300 specialist and two hospitals in Clark county participate in Project Access, giving free care to a pre-determined number of patients each year.
Four employees, and multiple volunteers devote time to screening, processing, and guiding referred patients through the steps toward free specialty care.

For Physicians who want their patients enrolled in Project Access
Patients must meet the following to qualify
- A Medical Referral Form along with chart notes faxed to 360-313-1381
- Medical need is within the Medical Scope
- Live in Clark County
- Between 18 and 65 years of age
- Does not have health insurance or access to insurance
- Lives at or below 200% of the Federal Poverty Level guidelines
Medical Referral form - If you want your patients to be screened for enrollment into Project Access just fill out this form and send pertinent chart notes and we will contact the patient within 2 weeks if their medical referral is approved. Click here for the form.

Medical Scope – These are the services that we can assist patients with. We exclude emergency situations and chronic care. Click here for form.
For Physicians seeing Project Access patients
Follow up Form – send this form after a visit with a PACC patient and we can connect the patient with services the patient needs outside your office Click here for form.
For imaging and surgeries we get the authorization at the hospital of your choice Legacy Salmon Creek or Peace Health Southwest
For labs just send the orders to Legacy or Peace Health SW let us know when the patient will be going – we will make sure the patient is not billed
Prescriptions – Fax the script to 360-313-1391 attn: Project Access. We will fill the script for the patient at the Free Clinic of Southwest Washington for FREE! Patients Rights and Responsibilities – the basic rules are
- Go to your appointments (two no shows and we will dis-enroll the patient)
- This card is only for appointment Project Access coordinates for you
- Emergency visits are not covered by Project Access
