Background Check

The Free Clinic of Southwest Washington (FCSW) conducts background checks for all new volunteer applicants in cooperation with our partner Vancouver Public Schools (VPS). VPS will screen FCSW volunteers in the same way they screen their own volunteers. Please download and complete this attached pdf file, then return it to the FCSW Volunteer Office at the time of your application to volunteer. Health professionals who are actively licensed, registered or certified with the Washington State Department of Health, have already passed a background check, and therefore are not required to repeat the process.
