Volunteer Applications

Our applications are based on your licensing status.  Our online system uses excellent security protocols to ensure all your information is protected.  Applications take approximately 10-15 minutes to complete.

    • Application - Health Care Professional 
      Select this application if you are applying to use your Washington State Department of Health *active* health-professional certification, registration, or licensing in a volunteer capacity.

    • Application - Health Care Professional - other 
      Select this application if you are applying to use an out-of-state health-professional certification, registration, or licensing (including national registries) in a volunteer capacity.

    • Application - Volunteer (non-clinical)
      Select this application if you are applying to volunteer in a capacity that does not require health-professional certification, registration, or licensing.

You will receive an automated email response once you submit the application, and we will contact you within a week or two to complete the registration process.  If you have any questions about the application process, please call the Volunteer Office at 360.313.1389 or 1399.  We are happy to help in any way we can! 

Volunteer Positions

There are always openings for doctors (and other providers), pharmacists, dentists, hygienists, optometrists, ophthalmologists, and immunization nurses.  All other position openings vary. Please determine if your desired position is open before applying since the applications list only current openings.handshake

      If your desired position is currently full, we can place you on a wait list for future openings. (Please make this request in a note section of the application.)  

Wait....there's a wait-list?
Even though we always need more providers, it is a testament to the spirit of this community that we have a wait-list for some (not all!) of our other volunteer positions.  We are grateful for the many enthusiastic people who wish to help.  If your desired position is currently full, please consider applying anyway, and we will do our best to find you a placement as soon as possible 

The process to become a volunteer

  1. Submit the online application.

  2. Complete and submit the criminal background check, including the Volunteer Agreement. (WA State professionals exempt.)

  3. Physician Assistant's will complete and submit the practice plan.
  4. View the Orientation Video.
  5. Review the Volunteer Handbook.

  6. Interview with the Volunteer Manager, and

    1. Sign the Patient Confidentiality Agreement.
    2. Provide photo identification.
    3. Sign the Declaration of Liability Insurance (only Physicians & Dentists)
    4. Tour the agency.
  7. Interview with the Program Manager, and
    1. Review written job description.
    2. Tour work environment and tools.
  8. Official offer to join volunteer team.
  9. Schedule first shift (1-2 shadow shifts for non-professional volunteers)
  10. You are now a fully-fledged member of this terrific team! You may begin self-scheduling online.
