Licensing
Licensed Volunteers
Free Clinic of Southwest Washington (FCSW) recommends that all health professionals be licensed (certified, or registered) with the State of Washington Department of Health (DOH).
Equivalencies
There are some equivalencies, most notably for credentials with national registries. If you are not licensed with the Washington DOH, please contact the volunteer office at 360-313-1399 to discuss equivalencies in more detail.
Free License Renewal
Health professionals who use their Washington license solely for volunteer work with underserved populations are eligible for a free license renewal. This includes not only retirees but professionals who work in other states (such as Oregon).
The Office of Community and Rural Health will pay the associated license renewal fees and forward your renewal paperwork to the relevant licensing department for processing.
Please note, volunteers are responsible for the cost of the inital license and any late fees or external fees such as testing or CME.
Step-by-Step Process:
1. Download and complete the License Renewal Form applicable to your professional license, or use the renewal you have received in the mail from DOH.
2. Download and complete this document “Certification to Use WA License only for Volunteer Work”
3. Send all documents to:
